Bus Stop Survey

If you want to request a new bus stop or a bus stop change, click on the link above to complete an editable  bus stop survey form. If you are filling out the form electronically, you MUST use Adobe Reader.  PDF Expert and other Apple or 3rd party apps corrupt the file, and we will not be able to read your survey submission.  Fill out the  requested information on the form and return via fax, e-mail or U.S. mail to the Transportation Services Department. Once received, administrative staff will evaluate your request as quickly as possible, but not longer than 45 days from receipt of the form, (in the order which the surveys are received).
