| Permission Slip | In order to attend an extracurricular school activity, other than a field trip, a student needs written permission from a parent or legal guardian. A blank form is available below. Field trip permission slips are available from your student's team at the time of the trip. |
| Absence Note | Before your son or daughter's absence from school can be excused, state law requires a written note from a parent or legal guardian explaining the absence. For your convenience, a blank form is attached below. |
| Power of Attorney | For school decision-making purposes, if you leave your children in the custodial care of someone other than their parent or legal guardian, a completed, notarized power of attorney is required. |
